Job summary

This is an exciting opportunity to develop your assessment skills and gain knowledge in a new field of healthcare. The role will involve assessing, reviewing and case managing clients with regards to their health needs, welfare and provisions of care, whether in a domestic setting orresidential placement. Our team is based in the heart of Bristol covering the BNSSG demographic across South Gloucestershire, Bristol and North Somerset.

Main duties of the job

We are work flexibly to undertake face to face assessments for our target population. We deliver person centred assessments of need and subsequent provision of health care if found eligible under the National Framework for Continuing Healthcare. The post requires the applicants to be fully mobile for the role and hold a full UK driving licence.

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Further training or significant experience in project management, financial management or supporting change management processes
  • Experience and understanding of evaluating and measuring the performance of health services.
