Job summary

In this role you will be responsible for ensuring Stockport's community services are commissioned to the highest standards to improve health care outcomes for the Stockport population. To deliver on the aims the NHS Long Term Plan and Stockport CCG's Strategic Plan.

Main duties of the job

We are looking for a pro-active and passionate candidate who can help our health economy to further improve on care delivered in the community and ensure that it is aligned to primary care networks and neighbourhoods. As a commissioning lead you will play a key role in ensuring that the community services deliver positive outcomes for our population, address health inequalities and are well embedded in the neighbourhoods. You will take responsibility for ensuring community services are specified with clear outcome measures and KPIs in the community contract with the Foundation Trust and other contracts with community providers and that outcomes are regularly monitored.

You will be working as part of an out-of-hospital commissioning team and with colleagues in social care, housing and public health. Areas of work are related to the management of long-term conditions, frailty, dementia and end of life care and other areas that benefit from integrated care delivery.

About us

In this role you are expected to represent the CCG in various multi-agency groups in Stockport and in strategic and clinical networks in Greater Manchester relevant to community services. You will also have a line management responsibility for a small team of commissioners.

Lead on a wide range of transformation and improvement projects from initiation to business case and on to implementation and delivery

Apply the commissioning cycle for all programmes and services within the portfolio

Contribute to operational and strategic planning processes

Commission services across integrated pathways of care based on best evidence and offer best value for money

Be responsible for the delivery of performance targets including financial balance, national and local targets

Responsible for presenting options and proposals to stakeholders on key commissioning plans, including patients, carers, service users, GPs, politicians and the general public

Negotiate changes to services and contracts

Assess the impact of the national planning guidance each year on their portfolio area and respond to political developments, nationally or locally and interpret the impact on the commissioning plans

Qualifications

Essential

  • Degree or equivalent level in management or clinical area
  • Educated to masters level or equivalent level of experience of working at senior level in specialist area

Knowledge & Experience

Essential

  • Evidence of significant experience of senior management within the NHS, working at a middle or senior manager level.
  • In depth experience and knowledge of working in NHS, understanding the national structure and how organisations interact.
  • Experience of designing and delivering change projects
  • Detailed in depth knowledge and experience of commissioning in an NHS setting, with one particular area of expertise in commissioned services, ie, mental health, urgent care, etc

Skills & Attributes

  • Ability to work to tight deadlines with diplomacy and work under pressure.
  • Ability to build effective relationships and work with staff at all levels Enthusiasm, drive and the ability to cope in challenging situations
  • Proven project management skills
