The NHS Continuing Healthcare team in Surrey covers Surrey Heartlands and Frimley Clinical Commissioning Groups part of Surrey Heartlands Integrated Care Board.
Our team's office is based in Epsom, Surrey with agile working conditions.
As aClinical Assessoryou will be responsible for theassessmentof Continuing Healthcare patients. You will use your knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.

Main duties of the job
The Continuing Care Team is responsible for delivering a service across Surrey. The county is divided into 4 Localities, co-terminus with Local Authority and Acute Hospital areas aligning with Surrey Heartlands CCG and Frimley CCG.
The post holder will work within the Continuing Care team which has responsibility for the full range of provisions relating to Continuing NHS Health Care and Funded Nursing Care. This includes the referral process, co-ordination of assessment of individuals, ongoing review, negotiating with care homes and allocation of resources for the eligible individuals and the appeals process.
