Job summary
This is an exciting opportunity to join the Surrey Heartlands Medicines Optimisation Team.This is a new role which is available initially for a one - year fixed term period, with the intention to review longer term.
We are looking for a forward thinking project manager to work with our Community Pharmacy Clinical Lead, to optimise the use of community pharmacy services within Surrey Heartlands. This role will support the integration of community pharmacy services, enabling in particular, the range of clinical services included within the Community Pharmacy Contractual Framework to be fully utilised in order to help manage overall demand in general practices and urgent care settings.
As the NHS enters a period of recovery following the Covid pandemic, community pharmacy is well placed to support the system in opening up access to a range of clinical and preventative services and supporting actions to mitigate against health inequalities.
For this to happen equitably across Surrey Heartlands expert project management to support the roll out of the community pharmacy clinical services is critical to realise the full potential of the nationally agreed contract for local populations. Main duties of the job
Given the breadth and cross-system nature of community pharmacy clinical services, this role has been developed to undertake implementation actions, to enable the roll out, uptake and embedding of new and recently commissioned clinical services.
The role will focus on integrating community pharmacy clinical services (e.g. GP and A&E referrals into the Community Pharmacist Consultation Service (CPCS), Discharge Medicines Service (DMS), smoking cessation service, Hypertensive case-finding service) into PCNs and the acute sectors, and increasing referrals into pharmaceutical services from these sectors where needed. As such the post-holder will be required to lead on integration activities to promote community pharmacy's role and the importance of pharmaceutical services. This will include the development of associated policies.
You will be responsible for leading the implementation and roll out of the Community Pharmacy Independent Prescribing Pathfinder sites and will provide professional support to the community pharmacists participating in the pilot.
The post-holder will work alongside the Community Pharmacy Clinical lead, South East Regional Chief Pharmacist and South East Regional Integration Lead.
