Job summary
The post holder will be responsible for line managing a Continuing Healthcare (CHC) clinical team.Supporting the CHC Review and Assessments Manager and taking responsibility for the management of the day to day work assigned to the clinical service line. Ensuring that the clinical team, when carrying out their duties, work in accordance with the latest statutory requirements, in particular The National Framework for NHS Continuing Healthcare and Funded Nursing Care and that the practice is in line with the Good Practice Guidance revised 2012 and within the Code of Professional conduct NMC April 2015.o Manage the Team's, including developing and managing the team's business plan, progress and reporting risk and issue management.o Liaise with appropriate functions within the ICBo Case management across all age Continuing Healthcareo Take a lead in the coordination of training & development and recruitment activity across the Team.o The above is only an outline of the tasks, responsibilities and outcomes required of the role. Main duties of the job
o Contribute to performance improvement, taking a lead for identified areas where agreed. Responsible for line management of the Continuing Healthcare (CHC) clinical team and providing support.o Maintain and improve quality of assessments/ Decision Support Tool by clinical staff and maintain consistency of recommendations.o Ensure that the CHC team use national and local assessment tools appropriately and adhere to the lead times. Coordinate/ participate /support the assessment process by the multidisciplinary team to determine the eligibility as set out in the National Frameworko Ensure that the CHC team understands the importance and takes responsibility for explaining to patients, families, health and social careo colleagues and care providers, so that the process is transparent, robust, clearly understood and involves the individual, advocate or family members.o To undertake assessments and reviews, maintaining appropriate written records and communicate information as required with all relevant parties whilst maintaining appropriate confidentiality of the patient record.o Provide coordination of and participate in relevant working groups and provide project advice, expertise and support where requestedo Work with members of the Team to investigate the causesof any variance from target/plan and proactively contribute to the implementation of solutions.o Management of a risk and issues tracking mechanism and its proactive resolution and escalation processes.
