Job summary

NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs.

Main duties of the job

This dynamic CHC role will operate within a fast paced, supportive environment where the patient is at the centre of everything we do. The team are looking for a self-motivated, organised, driven candidate, able to adapt and cope with changing demands. The Administration Team Leader will support the Operations Manager to provide a modern, effective, and customer-focused administration service. The post-holder will manage a team of administrators that support Nurse Assessors through the eligibility / commissioning process. They will have a sound understanding of the relevant national and local frameworks and procedures.

The team act as first point of contact for the department, triaging enquiries, dealing with routine and specialist queries in a friendly and professional manner.

Job description

Job responsibilities

The Administration Team Leader will support the Operations Manager to provide a modern, effective, and customer-focused administration service. The post-holder will manage a team of administrators that support Nurse Assessors through the eligibility / commissioning process. The work-plan sets out several priorities including we will:

Work with provider partners to further improve the CHC eligibility assessment process.

Demonstrate (quarter on quarter) that more clients experience high service quality and a positive customer experience.

Work closely with the Clinical Team to put in place an action plan to improve and sustain the quality of information received from referrers (including: CHC checklists, fast-tracks and retrospective applications).

Ensuring that delegated business processes are followed accurately / consistently.

Reporting aspects of business operations (e.g., alerting delays in triaging CHC referrals and outstanding case reviews) and escalating issues to the appropriate senior member of staff.

Measure and report compliance with CHC National Framework KPIs, e.g., timeliness of eligibility decision making.

Ensure data protection procedures and all areas of information governance are adhered to and managed to the highest professional level. Ensuring customer information and other personal information (e.g., staff records) is managed appropriately.

Day to day management of the Administration Team.

Line manages administrative staff (including bank / agency staff) as designated.

Setting out objectives and managing performance.

Supporting recruitment / retention and staff development.

Ensure ICB policies are implemented.

Undertake General Office management duties.

Accuracy of data in the CHC Patient Administration Systems (Caretrack).

Support Nurses Assessors as required.

The team act as first point of contact for the department, triaging enquiries, dealing with routine and specialist queries in a friendly and professional manner. Communicating relevant information to stakeholders (including the public, patients, relatives, NHS professionals, adult social care, nursing homes and other providers), referring to Nurse Assessors / Case Managers as appropriate, taking messages and signposting to the relevant departments/person. The team support logging and processing referrals, updating the database during the eligibility assessment process, organising multi-disciplinary teams meetings, taking minutes, processing eligibility decision letters. Supporting the Case Managers and commissioning process, arranging reviews and post decision letters.
