An exciting opportunity for an exceptional leader to make a difference to those people and communities across Devon, with complex health needs.
Working closely with the Chief Nursing Officer and the Deputy Director Market Development and Transformation, the post holder will play a pivotal role in the AACC transformational change plan with focus on service restructure; the procurement and implementation of a new digital case management system as well as the reinvigoration of the market development function within the ICB.
The post holder will be responsible for individualised commissioning including the joint strategic leadership and operational delivery of All Age Continuing Care services whilst providing expert clinical leadership for hospital discharge and Enhanced Health in Care Homes programmes, together with input to quality improvement and professional development in the sector.
Main duties of the job
As a member of the senior ICB team, the role would demand tactful management of system relationships and partnerships to create a culture for improvement and raising of professional standards. Driving positive working relationships between the ICB and provider collaboratives and a range of health and care organisations; as well as the independent and third sector.
