We are pleased to have a secondment opportunity for twelve months to lead on the day-to-day operations of NHS CHC.
Your expert knowledge and skills of NHS Continuing Healthcare will take the team to the next level as we focus on ensuring that our NHS Continuing Healthcare structure delivers the person centred, outcome focused care that we are developing as our delivery model. You will be responsible for delivering the day-to-day operations of CHC. This includes considering individuals for NHS Continuing Healthcare and NHS-funded Nursing Care, Fast Track and all other areas of the National Framework for NHS Continuing Healthcare.

Main duties of the job
The post holder will be responsible for the operational delivery of NHS CHC service on behalf of the Associate Director of Quality in Care and is required to provide, leadership and management to the CHC clinical team. The post holder will have responsibility and accountability for performance of the adult NHS Continuing Health Care portfolio to ensure the service delivered meets the statutory requirements of relevant NHS Frameworks and key performance indicators.
The post holder will be responsible for maintaining and further developing a culture of high performance, ownership and accountability where safety, quality, excellence in patient outcomes and value are consistently delivered in line with national and locally agreed standards. The Head of Continuing Healthcare will have accountability for the development and training of employees, to ensure that the CHC team has the necessary skills, capability and capacity to deliver and to actively promote the values of the service.
The post holder will be required to maintain and develop systems to allow for accurate reporting and monitoring of activity and spend.
The post holder will be required to provide professional nursing advice and to deputise for the Associate Director of Quality in Care as required.
The post holder will develop effective strategy and operational policies for promoting innovation across Norfolk and Waveney. Working closely with colleagues across the sector, ensuring coherent strategy.

- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent
