Job summary

Population Health Management is an ambition in Norfolk and Waveney 's Joint Forward Plan.

Our aim is to enable all people to stay healthy by predicting and planning for health and care needs before they happen, and ideally preventing them if we can. By working together with partners across the NHS and other public services in Norfolk and Waveney we can make an even bigger difference to many of the factors that affect our health and improve the health outcomes for our population.

This role will lead this ambition alongside colleagues in the programme team.

Main duties of the job

  • The post holder will act as a Specialty Advisor for Norfolk & Waveney Integrated Care System.
  • In this leadership role the post holder will be responsible for contributing to Population Health Management:
  • Providing specialty expert knowledge and advice for an identified system wide transformation programme or priority clinical and/or care area for example learning difficulties or population health management.
  • Leading transformation and service re-design in one or more of our ICS Transformation Programme areas.
  • Leading the coordination of responses to population health needs at system level, and with the place based teams as appropriate .
  • Providing professional advice within the system.
  • The post holder will work across Norfolk and Waveney multiple partner organisations to provide strategic, clinical and care leadership for the delivery of specific areas of work, as outlined within the role description and the applicant pack.
  • Over time, you may be invited to rotate between roles to share or broaden your leadership experience.

The role is part time, requiring one session per week (one session is 3.75hrs).

Job responsibilities

  • Work proactively across organisational boundaries within the Norfolk and Waveney Integrated Care System and across multiple partners to improve population health outcomes and the way services are delivered
  • Operate effectively in a flexible and demanding environment and proactively engage with stakeholders
  • Communicate to proactively build good working relationships; provide information and advice to a wide range of internal and external stakeholders
  • Maintain open and proactive two-way communication with specified line manager i.e. Deputy Director of Public health/ Associate Director of population health management, to discuss and update on priorities and programme developments. (Overall accountability remains with the ICB Executive Medical Director)
  • Drive and challenge each key working relationship to innovate reforms to achieve agreed objectives
  • Nurture key relationships with a range of individuals including other care professionals, clinicians, programme leads and patients/service users across health and care sectors
  • Identify and work with the relevant individuals in the health/care leadership of the assigned area of work e.g., appropriate professional groups, users, carers, key organisations and staff
  • Key working relationships will be with the Senior Population Health Management Manager, other PHM and health inequalities/inclusion group specialty advisor(s), as well as with colleagues from digital, business intelligence, public health and across primary care
