Job summary

*** Opportunity for six month secondment to join the Diabetes Clinical Programme Team. One post is available as either Clinical Programme Manager (AfC 8a) or Change Manager (AfC Band 7). ***

The NHS Long Term Plan (2019) sets out a range of improvements for those people at risk of Type 2 diabetes and living with Type 1 and Type 2 diabetes, including:

  • Type 2 prevention continues to be a priority - Healthier You: NHS Diabetes Prevention Programme.
  • Access to diabetes professionals is crucial for optimum treatment and care - diabetes inpatient specialist nurses (DISNs) and multi-disciplinary footcare teams (MDFTs)
  • Self-management has a key role in 'upstream prevention' - structured education for patients.
  • Low calorie diets may be a potential treatment options for Type 2 diabetes - national pilot programme roll out.
  • Diabetes prevention, treatment and care is going digital - flash glucose monitors for eligible Type 1 patients, pregnant women offered continuous glucose monitoring, digital options for Healthier You and structured education.

This role's focus is to lead a small team to deliver the Operational Plan for 2023/24 and support development of plans for 2024/25.

Main duties of the job

The key objectives are to ensure a system wide approach to implementing and developing transformation changes and clinical pathway improvements within all Norfolk and Waveney ICB commissioned provider services for diabetes to deliver the NHS Long Term Plan.

In collaboration with colleagues within the ICB and Provider leads the post holder will 'hold to account' and monitor the commissioned services for their improvement and outcomes related to the above.

The post holder will provide specialist advice to the commissioning organisations, and be a specialist advisor to all staff and related agencies. This will include the provision of project, programme and assurance reports on progress with changes they are facilitating on.

The post holder will provide to the contracting team specialist advice to enable the efficient management of relevant contracts achieving good patient outcomes and value for money.

The post holder will be aware of guidance from the Department of Health NHS England and other sources of best practice.
