Job summary
NHS North East North Cumbria ICB is a key part of
the NHS in our region, working to improve the health of our local population,
and ensure that all patients can easily access the care they need quickly and
easily when they need it.
There is a requirement for Integrated Care board to maintain a Dynamic Support Register (DSR). This is to identify people with a learning disability, autism or both who display, or are at risk of developing, behaviour that challenges, and/or mental health conditions who were most likely to be at risk of admission to a mental health hospital.
The DSR lead will be expected to be a local champion for autistic people & people with a learning disability. This role is to work alongside the Designated Clinical Officer, in the continued development of the DSR. Currently within Tees Valley, the DSR is closely linked with the Local Key Worker Service that is provided by Daisy Chain.
Main duties of the job
As part of this role the successful applicant will need to be able to form effective working relationships with both then Key Worker Service, Providers, Local Authorities, Education and Parent Carer Forums. Within Tees Valley the ICB also operated a Pre-DSR process, where a child / young person does not need to have a diagnosis of a Learning Disability or Autism to be placed onto a register and a Multi-Agency approach implemented. This pathway is currently in its infancy and the successful post holder would be required to support the Designated Clinical Officer in the development of this component of the register. The post holder will be good at working autonomously and in a team, showing high levels of self-motivation. The ideal candidate must also be able prioritise all work and adhere to tight deadlines, managing completing priorities. They will also have a track record of succeeding managing competing priorities, engaging, and collaborating with a range of stakeholders. One of the key components of this role, is to provide high quality and comprehensive leadership to the Dynamic Support Register and support the development of the systems and processes that currently and provide ongoing support and structure to the DSR.

- Educated to masters level or equivalent qualification or experience, which could include in depth and post graduate study or research in an area relevant to the role
- Highly developed and specialist knowledge in relation to learning disability, autism and mental health including knowledge of relevant legislation, local and national policy and good practice guidance
- In-depth and specialist knowledge and understanding of the Children and Families Act 2014, Care Act 2014, Transforming care agenda, continuing care frameworks and SEND Code of Practice 0-25 (2014), in addition to other relevant legislation, local and national
Experience
