Job summary
An exciting opportunity has arisen within the Northeast and
North Cumbria Integrated Care Board (ICB), Continuing Healthcare (CHC) Team,
covering Newcastle and Gateshead. We are looking for an experienced,
self-motivated individual who has the relevant transferable skills to join us as
a Case Manager. You will be working
within an integrated team of both Nursing and Social Work professionals based
at Ridley House in Gosforth which is also the location of our Nurse Assessor,
Appeals, Complaints and Business Support teams.
Main duties of the job
The successful applicant will be a Social Worker registered
with Social Work England or a Registered Nurse with current NMC registration
who can demonstrate significant post registration experience.
We are seeking a dedicated, motivated, and experienced
individual with a high level of emotional intelligence, excellent
communication, and interpersonal skills and that you able to work to stringent
national guidelines. You must be flexible and innovative with excellent
organisational skills. You will be required to work with a wide variety of
stakeholders, including patients and public, local GPs, Provider Trusts,
Voluntary Organizations, the Local Authorities, Public Health, and the ICB.

Staff are currently working a hybrid model of office,
