Job summary

The ICB Regional Learning Disabilities and Autism team would be keen to receive applications from experienced commissioners and / or case managers who want to make a difference for people with Learning Disabilities and Autism. These posts offer the opportunity to work creatively and co-produce better outcomes for people, with people and their families. A passion and determination to make a difference is essential.

Learning Disability and Autism is one of six priority workstreams across North East and North Cumbria Integrated Care system. Case and Commissioning managers are key members of the Workstream senior leadership structure.

The post holders will work alongside the Regional Manager, Clinical Champion and other colleagues as part of a regional team to deliver the NENC Regional Learning Disability and Autism Programme Plan on behalf of the ICS.

Main duties of the job

The Regional commissioning / case managers will be responsible for leading on the development of actions and programmes across the NENC System to improve the outcomes for people with Learning Disability and Autism. Specifically, to make a step change, to increase quality community provision, to enable discharge and reduce inpatient numbers.

Co-ordinate information and intelligence to effectively contribute to the strategy for commissioning care, support and accommodation for people who have Learning Disability, and Autism and complex needs. This involves collaborative working across public and independent sector.

Have a sound understanding of Transforming Care agenda and experience within hospital / community settings.

Have the knowledge and skills to challenge professionally and critically to ensure the best outcomes for people with Learning Disabilities and Autism and their families.

Have excellent communication and influencing skills, be flexible, adaptive and have a positive attitude.

  • Experience in working with people with complex needs in a hospital and / or community setting.
  • Evidence of resilience in a range of complex and demanding situations.
  • Experience of developing comprehensive commissioning risk assessments and associated management plans.
  • Knowledge of CTR / CPA process.
  • Experience of working with multi-agencies.

Desirable

  • Experience in research and audit.
