We are seeking to appoint several registered nurses to support the implementation of the
Newcastle Career Start Nursing Programme (CSNP). Hours of work will be apportioned between supervised and unsupervised clinical practice
within an allocated cluster of general practices (1 WTE CSN supporting clusters of practices
with a combined patient list size of circa 30,000) and within structured university and ‘in practice’ professional development programmes.
The post will offer the successful candidate an opportunity to gain knowledge and skills working alongside an experienced nurse mentor within an assigned clustering of practices, providing clinical services according to the health needs of the population and appropriate to his/her level of experience.
The successful post holder will be aligned to a PCN of general practices, each with a combined patient list size of circa 30,000.

• Chronic disease management – ranging from provision of structured clinics, supporting self- management through to management of people with complex co-morbidities
• Mental Health Work collaboratively in clinical practice and provide additional nursing support for their assigned cluster locality to: - Assess, plan, implement and evaluate nursing care to meet the needs of individual patients at a level appropriate to the trainee’s knowledge and skills.

The CSNP is a Primary Care developmental programme with the successful candidates undertaking a 2½ year individually structured training programme aimed at First Level registered Nurses wishing to gain the skills and experience required to work as a Practice Nurse in Primary Care.
It is expected that the CSPN will remain with the locality they are assigned to for the duration of the programme. However, it may be necessary to alter working arrangements and the scheme reserves the right to do so. Although the post holder's time will be divided equitably amongst all of the practices each PCN contains one identified practice who will act as the lead or employing practice for the duration of the 2.5 year post. An external scheme co-ordinator and clinical supervisor will provide the post holder with clinical supervision and will act as a clinical and management champion for the scheme.

Protected time will be allocated for course attendance but home study will also be
required.
• Expand their portfolio of skills over the programme period and continuously
increase their scope of practice within an unsupervised autonomous clinical role.
• Be a proactive advocate for the Career Start Practice Nurse Scheme working with
their nurse mentor and scheme co-ordinator to continuously develop the scheme
for themselves and future cohorts.
• Actively engage in clinical supervision and reflective practice.
• Be an ambassador for practice cluster working by demonstrating flexibility and
adaptability to be a pro-active member of more than one primary health care team.
