We are seeking a skilled and compassionate Band 7 Complex Care Case Manager to join our Complex Care and Case Management team. In this pivotal role, you will focus on preventing admissions under the Mental Health Act and expediting safe discharges. You will work closely with multidisciplinary teams (MDT) to ensure personalised support is in place for effective discharge planning, chair Care (Education) and Treatment Reviews (C(e)TRs), and complete Learning Disabilities Mortality Review (LeDeR) processes.

Main duties of the job

Key Responsibilities:

  • Preventing Admissions: Work proactively to prevent unnecessary admissions under the Mental Health Act.
  • Safe Discharges: Expedite safe discharges from inpatient settings by ensuring all necessary support and resources are in place for service users.
  • Multidisciplinary Collaboration: Collaborate with MDTs, including healthcare professionals, social services, and other relevant agencies, to develop, implement and review personalised packages of care and support.
  • Chairing C(e)TRs: Chair Care (Education) and Treatment Reviews to ensure that care plans are comprehensive, person-centred, and effectively address the needs of individuals.
  • LeDeR Reviews: Conduct Learning Disabilities Mortality Reviews (LeDeR) to identify areas of improvement and contribute to the enhancement of care quality and safety.
  • Documentation: Maintain accurate and up-to-date records of all interactions, assessments, and care plans in compliance with NHS standards and regulations.
  • Training and Support: Provide guidance, training, and support to junior staff and colleagues to promote best practices in complex care management.
  • Quality Improvement: Participate in quality improvement initiatives and contribute to the development of service policies and procedures.
