Job summary
NHS North East North Cumbria Integrated Care Board is a key part of the NHS in our region, working to improve the health of our local population, and ensure that all patients can easily access the care they need quickly and easily when they need it. We are seeking experienced qualified nurses to join our All Age Continuing Care Service.
Main duties of the job
We have exciting opportunities available within the ICB across the North East and North Cumbria ICB region covering south area covering South Tyneside and Sunderland, Durham and Darlington and Tees localities. The posts are open to nurses with RGN, RNLD or RMN registrations. Reporting to the All Age Continuing Care Clinical Lead Nurses, you will be part of a dedicated team working within the health and social care system. Committed to high quality patient and family experiences you will be working collaboratively alongside other partners and stakeholders to ensure individuals needs are appropriately met. You will be responsible for the specialist Continuing Healthcare and Children's Continuing Care assessments, reviews and case management of the health needs of individuals either in their own homes, residential, specialist accommodation or nursing accommodation and act as a point of contact, referral and advice, completing regular reviews for health funded individuals and ensuring compliance with NHS National Framework Guidance.
You will be expected to have excellent communication skills, be able to work with families, carers and other stakeholders in a complex and challenging but supportive work environment. Whilst experience of involvement in the All Age Continuing Care processes is desirable, this is an excellent opportunity for nurses to increase their knowledge and understanding and enhance their skills whilst being part of an experienced and supportive team.
