TEAM / ROLE PURPOSE

Doncaster Place SYICB Transformation and Delivery Team. The purpose of this team is to work with health and care partners to transform services, always keeping the patient at the heart of what we do

ORGANISATIONAL CHART

LOCATION / BASE

SYICB currently operate using a hybrid working model; combining both home and office working. There is an office space based in each place across South Yorkshire for which all staff are entitled to work from.

Doncaster Place office space will be your base.

Main duties of the job

In order to be able to deliver the assignment brief we are looking for enthusiastic individuals who foster positive working relationships and can negotiate, problem solve and plan strategically. NHS South Yorkshire Integrated Care Board oversees health and social care for a population of 1.4m people. Job responsibilities

Working as a key member of the transformation, delivery and improvement team you will support the key functions of the commissioning cycle including analysis of need, service design and improvement with a focus on health inequalities. This role will be pivotal to the delivery of strategic developments and projects including development of project plans and collaboration with service providers across health and care,

You will be educated to masters level in a relevant area of academic practice or equivalent experience working at a commensurate band/level.

You will understand the commissioning function and its component parts and have experience of commissioning in a health or social care setting.

Demonstrable experience of project management and leading service change as well as experience of managing stakeholders are key requirements.

You should be able to provide and receive highly complex and sensitive information and be able to draw conclusions from a range of sources in both qualitative and quantitative information.

ROLE SPECIFIC OBJECTIVES

The post holder will support the delivery of specific work streams and programmes; the below are examples of the current prioritised workstreams for the team and your role will include, but will not be limited to:

Priorities as identified in the 1 Doncaster Plan for 24/25:

  • Supporting the review of the Better Care Fund recurrent and non-recurrent schemes, to identify opportunities to achieve more from our pooled funds
  • Supporting the review of Section 75s, 76s and 256s and the development of associated specifications where necessary
  • Supporting the review of Doncaster Place contracts with providers, identifying opportunities for savings in particular
  • Leading winter resilience across the Doncaster health and care system
  • Leading the annual planning round across the Doncaster health system
  • Supporting the delivery and future development of the covid vaccination programme

Key objectives / purpose of this role include:

  • Lead on elements of the Living Well Strategic Delivery Plan
  • Develop the living well operational delivery plans for 24/25 for specific areas included in the living well portfolio
  • Lead on specific projects and programmes across portfolio areas

Living Well portfolio areas include but are not limited to:

o Mental health, Learning Disability and Autism

o Cancer

o Long Term Conditions

o Planned care

o Urgent and emergency care

o Long term conditions and pathways

o Primary care

It is expected that the postholder will:

  • Focus on mental health, learning disability and autism and the development of key pathways ensuring plans are robust including winter plans
  • Have oversight of cancer and the development of the annual delivery plan
  • Have oversight of crisis alternative work, employment advisers and individual placement support led by other members of the team
  • Work with provider alliances and local partners to implement national and local priorities and existing contractual agreements
  • Have oversight of key contracts as they relate to the lead areas
  • Work with partners on embedding key projects to ensure integration and aligned governance (rough sleepers project and complex lives for example)
  • Seek innovative ways of utilising investment opportunities securing exit plans for non recurrent projects
  • Line manage and develop members of the team
  • Support other Senior Transformation and Delivery Project Managers and their workstreams as required to ensure a balanced workload across the directorate
