and care outcomes for our young people with Type 2 diabetes age 18-39 years in
our South Yorkshire population.
After a recent successful opportunity to access
NHS England funding for a 2-year, T2Day: Type 2 Diabetes in the Young
programme- an initiative for people with Early Onset Type 2 Diabetes, we now
have a fantastic opportunity for a creative and passionate Practice Nurse/ Nurse Educator with Special
Interest in Diabetes.
Main duties of the job
The T2Day programme is an exciting opportunity for SYICB to support improved care delivered via additional reviews focused on opportunistic completion of remaining care processes, optimisation of glycaemia, cardio-vascular risks and weight, contraception, and preparation for possibility of pregnancy, and supporting any unmet psychological or social needs.
This approach will aim to improve the outcomes of young people living with Type 2 diabetes in South Yorkshire, whilst reducing health inequalities. It will include action to reduce the occurrence of diabetes and improve long term management therefore preventing diabetes related complications, including addressing wider determinants of health. It will require partnership working across organisations.
The areas that will sit under the T2Day
diabetes programme are:
- NHS Diabetes Prevention Programme
(NDPP).
- NHS Type 2 Diabetes Path to Remission
Programme (T2DR)
- Structured Education and Healthy
Living with Type 2 Diabetes (a digital structured education programme)
- Treatment and Care- Completion of NICE
recommended care processes and Achievement of Treatment Targets.
The
ideal candidate will have excellent knowledge and experience of working in a
primary care setting and sound clinical knowledge
of people with diabetes as is a working knowledge of long-term conditions
and cardiovascular disease, and an awareness of national polices and priorities
that will impact our stakeholders.

This 12 month secondment post with a possibility
to extend for another year will support the T2Day: Type 2 diabetes in the young
programme of the South Yorkshire Integrated Care Board (SYICB). The programme has
been launched by NHS England as people aged 18-39 living with type 2 diabetes
are less likely to receive all their annual diabetes health checks. By ensuring
people receive these checks, the NHS hopes it will minimise the risk of people
going on to develop complications of diabetes in later life.
The ideal candidates for the role will have
knowledge and experience in the following areas:
- Communication
and relational skills
- Ability
to liaise effectively with senior stakeholders, managers, and clinicians across
different organisations.
- Candidates will also have excellent interpersonal skills and will be comfortable
working with a wide range of stakeholder organisations in GP practices,
community services, acute trusts, voluntary sector, and private sector.

be key to influencing change across multiple organisations, you should be able
to work flexibly, creatively and be open to new ways of doing things.
Main duties will include:
- To support the clinical Lead in creating a SYICB Diabetes factsheet to understand the wider impact of diabetes in young adults with Type 2 Diabetes and highlight effective interventions with plans to implement it.
- To work closely with clinical Leads to proactively identify young adults with Early onset Type 2 Diabetes in SYICB.
- You will work closely with clinical application team to support the design of T2Day Diabetes clinical page for clinical systems across each place and ensure appropriate coding is in place for the T2Day programme.
- The post holder will work closely with medical education colleagues to develop and deliver multi-disciplinary education/ training for primary care team, midwives, and other clinical staff and raise awareness of the T2Day programme.
- You will act as a resource for health care professionals and support primary care staff development, role model best practice, plus undertake bite size teaching and educational projects as required and assist in the design and delivery of T2day educational programmes.
- To establish link with key multidisciplinary teams retinal screening, obesity, and public health teams to raise awareness of the T2Day.
- You will be working closely with the clinicians and contribute to the development and implementation of clinical protocols and guidelines and will work towards developing a structured approach in diabetes care.
- To support the T2Day programme team and to ensure that the T2Day programme provides targeted intervention for each person under the age of 40 living with type 2 diabetes in SYICB, including additional reviews focused on completing proven diabetes care processes, managing blood sugar levels, weight management, preparation for pregnancy, and supporting any unmet psychological or social needs.
- They will contribute to the overall development of the programme, scoping new projects and innovations, responding to emerging knowledge and techniques, and ensuring that the T2Day programme is person centred, evidence based, accessible, needs led and integrated.
- Lead projects/provide support to projects as requested and/or agreed with programme manager.
- The post holder will participate and contribute to the audit or quality improvement projects and support the project officer to enhance the quality and effectiveness of the T2Day service.
- To work with the T2Day team to highlight or signpost the development of culturally appropriate T2Day services for patients from black and minority ethnic (BAME) groups and other minority / excluded groups.
- To support the Diabetes specialist midwife to standardise the implementation of a postnatal screening guideline and annual HbA1c for all women who have had a previous diagnosis of GDM in primary care.
- To support Diabetes specialist Midwife to ensure systems are standardised for follow-up of women with GDM in primary care with implementation of a call / recall system and development of a post-GDM care pathway in primary care.
- Ensuring every young adult with Type 2 diabetes age 18-39 has access to vital routine diabetes care as this is proven to be lower in this age group putting those affected at a higher risk of potentially devastating complications.
