Job summary
The post holder will lead and programme manage the work
being delivered in Barnsley, Doncaster, Bassetlaw, Rotherham and Sheffield by
the ICB Diabetes programme board. This comprises of the ICB leads; the
Clinical Network and providers in the delivery, implementation, commissioning
and management of all Diabetes Services. This includes Acute and Community
Providers and activity monitoring, services delivered via NHS England such as
the NHS Diabetes Prevention Program and the
Treatment & Care Programme.
They also will lead on prevention /long term conditions
projects that have interdependencies
with the diabetes programme delivery across ICB and at place e.g links with
CVD programme, population health management
. They will be
line- managed by the ICB Programme lead whom is the Lead CAT Manager at
Barnsley Place but will be supporting all the ICB Place leads to implement
the programme.
Main duties of the job
1.
Make an active and major contribution to the
transformation of diabetes services across South Yorkshire; actively leading on
the programme and project management and subsequent implementation for the SYB
ICB Diabetes Board and NHS England
2.
Lead the strategic planning of the programme
and projects, identifying interdependencies, and potential impacts on wider
organisation, resource requirements and building in contingency and adjustments
as necessary.
3.
Part of the role will be
to work with the ICB Clinical/Professional Lead for Diabetes , NHS England
colleagues including the Clinical Network,
as the day-to-day lead contact for the ICB diabetes programme. This
includes in relation to newly commissioned services, pilots/trials which can
improve service delivery and close liaison with all of the provider Leads,
respective Acute Trusts and Primary Care Networks .

Ensure patients and
public are involved in all aspects of
the programme delivery.
5.
Responsible for Implementing policies and service changes policy or
service changes for the NDPP and Diabetes Transformation programme impacting
across the place areas.

9.Ensure integration is designed into any service transformation and commissioning work
10.Lead, Co-ordinate and deliver the analysis, synthesis and presentation of appropriate quantitative and qualitative data required to support local commissioning development
