We are
looking for a focussed, driven and forward-thinking system changer who has a
real passion for making a difference to work with autistic people, or people
with a learning disability and their families and carers in a person-centred
way, to help bring their voices to life and achieve their outcomes. This
exciting role is suitable for a wide range of practitioners, qualified or
unqualified (with suitable demonstrable experience).
Successful
candidates will;
Have
relevant experience of working with autistic people and/or a learning
disability
Be
passionate about working co-productively and have experience of contributing to
personalised care plans
Have
a good understanding of working in partnership and developing positive
relationships
Be
assertive to appropriately challenge the systems and processes when they are
not working

innovative with the ability to think out of the box using creative solutions to
prevent further escalation,
family or placement breakdown and admission into an inpatient setting

Main duties of the job
The focus of the job role is to ensure the system is cohesively working together to ensure people, carers and families receive the right care and support, from the right person at the right time including:
- Providing emotional and behavioral support around complex needs, ensuring clear communication with all parties involved.
- Advocating in care discussions, maintaining accurate records of interventions.
- Building positive relationships with various professionals to prioritise home and community-based care.
- Troubleshooting and challenging practices to ensure appropriate and necessary support within the community.
- Providing evidence-based challenges to practices and decisions, identifying areas for improvement.
- Facilitating transitions into and out of inpatient settings, coordinating care across health, education, and social care
- Communicating sensitive and complex information regarding their care in an empathic and supportive way
