We have an exciting opportunity for a Qualified Pharmacy Technician to join us at South Yorkshire Integrated Care Board (SY ICB) as a Medicines Optimisation Technician within our award-winning team.

You will have the opportunity to help shape the medical care of the population of SY, working to remove inequality of access and outcomes and reduce the morbidity experienced by those with health conditions. You will assist us with cost saving and quality workstreams.

Main duties of the job

You will assist us with cost saving and quality workstreams by making agreed changes to patients medicines and providing practices and PCN teams with timely and accurate prescribing performance data, supporting them to manage their prescribing budgets whilst adhering to local guidelines and formularies.

You will have knowledge and understanding of the ICB Quality, Innovation, Productivity, Prevention (QIPP) plan and support its implementation at practice level. Also having knowledge of guidelines and policies developed in response to NICE or NHS England publications.

You will assist in information sharing relating to medicines, including national and local prescribing policies, prescribing committee decisions, and medicines safety issues.

And assist with investigation of medicines related safety incidents, CQC concerns and support the development of systems and processes.

You will support the provision of appropriate medicines optimisation education and training events for healthcare professionals and build relationships with practice and PCN teams to ensure effective communication on issues relating to prescribing and population health needs.

Job responsibilities

  • To utilise effective communication with patients/carers, provider colleagues, other members of the Medicines Optimisation Team, wider ICB, and stakeholders. The information may be of a complex or sensitive nature.
  • To communicate results of audits and other prescribing related work to ICB partner staff concisely, presenting information in a suitable format (e.g., written and/or verbal) and style for the target audience.
  • To ensure information is provided in a timely manner to the appropriate individual.
  • To attend and contribute to partner provider meetings and other prescribing meetings, on issues relevant to prescribing and medicines optimisation.
  • To communicate information and issues to line manager.
  • To assist with the monitoring and analysis of prescribing data at ICB and ICS level and offer support to partners which are outlying on key prescribing indicators.
  • To produce reports for ICS partners and the SY ICB.
  • Undertake audits and reviews of SY ICS partners to assess prescribing against local and national guidelines and provide an interpretation of the outputs to colleagues.
  • To assist in the collection and analysis of clinical data from patients notes to facilitate audits related to practice specific issues, ICB priority areas and local /national guidelines.
  • Planning and organisation of straightforward tasks, activities or programmes to be delivered to ICS partners or ICB colleagues e.g. training on a particular medicines topic, work modules in GP practices or care homes.
  • Planning own work to meet targets, some of which may be ongoing work and which may require the adjustment of the plan to be able to continue the allocated work
  • The role requires keyboard skills daily. Post holders may also be required to learn and demonstrate certain medical devices eg: inhalers, insulin measuring devices.
  • Working from a fixed workstation will be required for much of the day
  • To provide, if deemed necessary by the ICB, direct patient care and consultation related to health initiatives.
  • Provides advice and clinical technician services to practitioners and patients on the guidelines and protocols to improve the care of an individual, or groups of patients/clients in the care settings supported by the ICB.
  • To work with ICS partners towards the delivery of the medicines optimisation Quality, Innovation, Productivity and Prevention (QIPP agenda) and ICS IPMO agendas, implement and support local formularies and ICS traffic lights, and other prescribing objectives and strategies as determined by the medicines optimisation team. Feeds back to line managers and colleagues where improvements could be made across the spectrum of work encountered.
  • Postholder should observe their personal duty of care in relation to IT equipment providers and ensure protection of data held in line with the information sharing protocols.
  • To support with the training and induction of new and existing staff members.
  • To deliver agreed training to provider colleagues.
    • To make agreed changes to patients medicines and update patients records with details of the changes made and to update records in a timely manner at the time the intervention is made.
    • To work with members of the team to develop and implement data collection systems that will provide accurate and timely data.
    • To carry out timely and accurate information analysis and reporting on agreed areas of portfolio and present findings in an agreed manner.
