Job summary

The post holder will play a key role in supporting the Continuing Healthcare service in Sheffield. Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of clerical support and consistently deliver a patient-focused service.

The post holder will be required to work under their own initiative as well as working as part of the wider business support team and will demonstrate a willingness to help others and be a key member of the team with responsibility for specifically allocated tasks. The post holder will be expected to follow set processes, prioritise their own workload and demonstrate initaitive in order to fulfil the requirements of the post. Excellent interpersonal skills and communication skills are essential plus an ability to deal with highly sensitive and confidential information.

The post holder will be the initial point of contact in the office and must have competent ICT skills including word processing, database inputting and diary management skills.

The post holder must have a flexible and adaptable approach to their work in order to meet demanding deadlines and have the ability and resilience to regularly deal with a range of routine and non-routine administrative tasks as required to deliver an efficient and effective service.

Main duties of the job

This role supports specific functions within the wider business support team and includes:

Booking patient assesments and having direct contact with potentially distressed and anxious patients and their relatives, treating them with tact and dignity at all times

Work closely with the nurse co-ordinators/care managers in planning workload to ensure patients who are due a decision support tool, and their representatives/next of kin, receive correspondence and supporting information within agreed timescales.

At the request of the clinician obtain any supporting information from other professionals involved in the patients care which may be relevant to the forthcoming review. Ensuring these documents are stored effectively within existing patient information systems.

Using the database as a source of information,

o Ensure information held on database is accurate in terms of contact names and addresses, and review type.

o Generate invite to review letters and text messages to client representatives and care providers, as required using SystmOne templates.

Maintain nurses/electronic SystmOne diary to accurately reflect assessments booked for the nurse co-ordinators/care managers.

Ensure information systems updated once review completed to maintain accuracy of data entry and reporting functionality.

  • Accurately inputting onto the Continuing Healthcare database, maintaining data storage and archiving of data as required and in accordance with standard operational policy
  • Ensuring that data systems and databases are kept up to date to allow accurate and detailed reporting capabilities for both internal and external requirements
  • Ensuring that all paper files are collated and filed in a robust and effective manner and take responsibility for archiving documentation as required and in accordance with processes, including Independent review administrative work as directed and when required
  • To use own initiate wherever possible to ensure the work flow is maintained.
  • Undertake delegated actions on behalf of managers, eg chasing progress, producing basic reports, making follow-up telephone calls, collating information.
  • Produce reports via Excel regarding budget extracts, and providing an intermediate level of analysis of information. Extracting information into a reportable format.
  • Contribute during times of absence to the management and oversight of the team generic email accounts.
  • Provide general business support within the main CHC Business support team as required, full training will be provided.
