Job summary

The post holder will have a lead role working within the Individual Patient Activity (IPA) Team concerning the commissioning and reviewing arrangements associated with our adult complex client placements. The post holder will take a lead role in attending case reviews including, Care Program Approach (CPA), monitoring standards of care and treatment and liaising with statutory and non-statutory agencies in the repatriation of Lancashire & South Cumbria residents.

The post holder will have responsibility for operational & line management duties within their team. The post holder will work closely with adult social care colleagues and commissioning leads in the ICB and Local Authority in health developments and strategy and will develop close working relationships Local Authority leads.

The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

Main duties of the job

  • Deputise for Clinical Leads when needed and ensure the effective and efficient use of human, physical and financial resources in the absence of the Team Lead
  • Contribute to professional and policy making activities that impact on the team and other health care professionals.
  • Provide leadership and co-ordination across professional boundaries working closely with other clinicians within the same area of practice.
  • Possess an in-depth knowledge of national policy area and changes to policy relating to CHC. Be actively involved in local policy development and implementation of policy changes within IPA.
  • Ensure informed information is available on care and treatment as required to assist the commissioning
  • Investigate sources and reasons for referrals and to influence future referral patterns
  • Determine compliance with the ICB commissioning policies
  • Support the complex MDT meetings in ensuring the most effective care/treatment of patients in the most appropriate location
  • Monitor Delayed Discharges of Care and engage in call conferences with other health and social care professionals about delays.
  • Provide specialist expert knowledge of IPA to clinical and Social Care colleagues and safeguarding adults/Mental Capacity Act/ MHA advice and support.
  • Take a lead role in governance within the IPA Team including undertaking audits and providing reports as requested
  • Provide line management for the IPA Team
  • Monitor compliance with terms of authorised referrals

Job description

Job responsibilities

  • Attend Complex Client MDT meetings as appropriate which consider difficult to place clients/high cost Prepare and circulate reports relevant to meeting, including reports for Court and attendance at hearings where necessary
  • Understand and undertake DOLS assessments and necessary action to inform council colleagues
  • Use advanced clinical assessment skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable planning and treatment of patient care.
  • Manage a small caseload of complex IPA cases
  • Develop and use specialist skills and knowledge to deliver specialised advice and care.
  • Develop and maintain a case management system containing details of relevant complex Provide the first point of contact for internal and external enquiries about individual cases, preparing case reports for the consideration of the panels
  • Provide advice on clinical issues relating to individual cases where appropriate
  • Monitor compliance with terms of individual patient/client Service This includes awareness of expenditure.
  • Liaison with the Councils and Regional Secure Commissioning Team about placements, admissions reviews and
  • Work in collaboration where necessary with the ICBs Fully Funded Continuing Care/NHS Nursing Home Care Place based Lead Nurse concerning future need/provision of nursing home
  • Establish partnerships and networks with social services, voluntary sector and other agencies to ensure patients with complex continuing health care needs have access to all resources available. Work closely with all of these agencies to act as an expert, support and advise these teams as appropriate in the assessment, appropriate care and placement for patients.
  • Monitor and Review packages of care to assess changes of patients needs and costs.
  • Maintain clinical competency in a range of extended skills, medical knowledge as required, to meet the needs of the patients.
  • Make autonomous decisions about the management of clinical care based on assessment and differential diagnosis.
  • Work with other health care colleagues, voluntary and statutory organisations to ensure that the holistic needs of patients are met wherever possible.
  • To co-ordinate the interface between acute, primary, community and social care settings using the single assessment process.
  • Attend safeguarding meetings and contribute to decisions as required.
  • Attend Court of Protection as a witness as required and provide relevant documentation and detailed written reports as requested.
  • Where necessary together with the Service Manager/Team Lead IPA and work with legal representations of the organisation.
