A CLINICAL LEAD IS REQUIRED FOR THE INDIVIDUAL PATIENT ACTIVITY (IPA) TEAM WITHIN LANCASHIRE & SOUTH CUMBRIA ICB

IPA INCLUDES JOINT FUNDING, S117, LD & COMPLEX PHYSICAL HEALTH CARE PACKAGES IN A VARIETY OF SETTINGS

YOU WILL BE REQUIRED TO MANAGE A TEAM OF STAFF WHO COVER FUNDING FOR IPA ACROSS THE LANCASHIRE& SOUTH CUMBRIA FOOTPRINT AS WELL AS LIAISING WITH PLACE BASED CHC CLINICAL LEADS

ROLE IS FAMILY FRIENDLY AND OPEN TO ALL ALLIED HEALTH PROFESSIONALS & NURSES

The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.

Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will manage a team of professionals covering Discharge to Assess process and provide specialist advice to the IPA Advisors. The post holder will have responsibility for the operational and line management duties of the IPA Team in the commissioning of care packages in the community and in residential settings. The post holder will work closely with the Head of Service CHC & IPA

The post holder will work closely with acute trust & adult social care colleagues as well as commissioning leads in the ICB and Local Authority in health developments and strategy.

Main duties of the job

Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL as Clinical Lead for our IPA Team

The post holder will play a key role in supporting the All Age Continuing Health Care & Individual Patient Activity (IPA) service across the region.

The post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will be responsible for providing management and clinical leadership and overseeing a team of professionally qualified health professionals in the delivery of Individual Patient Activity and Deputising for the Head of Service as required

The post holder will work closely with adult social care colleagues in health developments and strategy and will develop close working relationships with Local Authority Learning Disabilities Team and Mental Health Team in relation to specialised commissioning of complex cases.

The post holder will actively contribute to the strategic planning of team projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building in contingency and adjustments as necessary.

The post holder will work with the Business Manager to ensure delivery and compliance with key performance indicators and performance targets.

The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

  • Responsible for the leadership, management & oversight of the IPA Team within ICB working alongside other leads within CHC teams
  • Responsible for ensuring that all clinical aspects of enquiries, disputes, challenges and complaints are managed and co-ordinated in a timely and appropriate manner in line with policy and guidance.
