Job summary

We are seeking an Ageing Well Programme Lead who will be responsible for the strategic leadership of the Lincolnshire wide Ageing Well programmes including Frailty, Palliative and End of Life, and Enhanced Health in Care Home.

These programmes will deliver the objectives outlined in the NHS Long Term Plan (2019), Lincolnshire Joint Forward Plan (2023) and System plan.

You will be responsible for the development and delivery of the Ageing Well strategies. The purpose of which is to bring together multiple providers from across the integrated care system to enhance proactive and preventative treatment models in the community, strengthen integration and interface and through this deliver improved patient outcomes and best use of system resource. In addition, the strategies will support the delivery of the systems' operational planning targets and address issues of variation, quality and patient experience. The Ageing Well programme manager will be responsible for the coordination of individual projects managed as part of the programme to ensure delivery of the agreed outcomes and benefits.

You will lead a team of staff as well as provide a leadership role to provider staff/team/resources assigned to the programmes of work.

You will be required to develop and promote partnership working across system stakeholders from health and care.

Main duties of the job

  • Provide assurance on delivery of the Ageing Well programmes and projects objectives and benefits to Board level within Lincolnshire Integrated Community Care (ICC) and ICB and regionally to NHSE.
  • Lead the Programme Steering Group using the governance structure to ensure that the work programme is appropriately monitored and managed so that the required change is delivered.
  • Proactively engage with regional and national teams to 'horizon scan' for new national and local guidance and policy changes that will impact on service delivery or performance with a view to updating the ICB and system partners and reviewing impact upon existing delivery plans and priorities.
  • Identify priorities and critical actions and develop detailed programme plans and critical paths which will deliver the programmes of work ensuring the programme plans are suitably ambitious, comprehensive and robust and in line with the programmes' objectives.
  • Ensure that the work programmes connect to the others within the Integrated Community Care programme, and link to those in other system programmes e.g CYP.
  • Manage these programme interdependencies and any conflicting interdependencies and unresolved issues within tolerance. Escalate unresolved issues to the programmes' SRO or Programme Steering Groups as necessary.
  • Deliver high quality business cases, aimed at delivering the objectives of the work programmes', including financial objectives, seeking authorisation through due governance processes.

  • Relevant Masters qualification at degree level or equivalent experience
