Based in Kirkham House, Worcester or St Owen's, Hereford.

Main duties of the job

The post holder will operate in the Business, Contracts and Commissioning function of Continuing Healthcare and will report to the Senior Contracts Manager. They will have responsibility for specific contracts and their performance to ensure delivery of the best possible healthcare outcomes which meet national and local priorities. They will also lead on a portfolio of provider contracts, to ensure value for money and quality of provision, and to maintain comprehensive oversight of commissioned services and contractual requirements that support performance management e.g. Quality Indicators/KPI's, Service Improvement Plans, etc.

The post holder will be required to generate and utilise assessment and review information through accessible care package templates that identify patient's needs in enabling management and support of providers. This will be done by working in partnership with the CHC's Services Clinical Team. The role will support the contracts' function but will also have a direct link into the Brokerage function in terms of provider contracts and support.
