We require a motivated and experienced registered nurse, ideally LD qualified or with significant LD experience, who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.

This is a key management role where you will lead a clinical team, providing expertise and acting as case manager for highly complex cases.

You will have knowledge of the National Framework for Continuing Healthcare (2022). You will also need a good understanding of the Mental Health Act (1983), and the Mental Capacity Act (2005). You will be working in partnership with our local authorities, clinical teams, and other stake holders therefore experience of multi-disciplinary working is essential.

A current driving licence and vehicle insured for business use or the ability to otherwise travel to a variety of locations will be required.

Main duties of the job

The post holder will be responsible for the day to day management of a small team of LD nurses. The post holder will also hold a caseload of complex cases and responsibilities will include:

Assessment & case management of patients funded by NHS Continuing Healthcare (CHC). This includes review of commissioned care to ensure that care is robust and effective, taking action where any quality or safety issues arise.

A key aspect of this post will be to provide professional leadership, and clinical expertise to other members of the CHC Team. This includes support to the Clinical Assessors, as well as to the Head of Joint Care Home Contracting & Procurement, Clinical Operational Leads, the Continuing Healthcare Panel, and others as appropriate.

It is expected that the post holder will contribute to the management and be part of the CHC Duty Rota. The CHC Duty role manages referrals through the CHC process, including those patients that have rapidly deteriorating conditions along with managing daily enquiries to the CHC Service and any case management issues that arise unexpectedly.

It is envisaged that the post will be instrumental in securing and ensuring the highest standards within CHC by maintaining standards within the CHC team and by working in partnership with care providers to improve the patient experience. The post holder will act as ambassador for CHC both within the ICB, and in their contacts with all other stake holders.
