Job summary

Coventry and Warwickshire Integrated Care Board are seeking a Health Inequality Programme Manager to join our inequalities team .

This post plays a pivotal role in improving the health outcomes for our local population by leading on the delivery of the System Healthcare Inequalities Strategy including evidence-based programmes and projects to tackle avoidable inequalities in health, working with a range of system partners including local public health teams.

You will also link with either Coventry or Warwickshire Geographical Care Collaborative working with multiple partners to shape and target delivery of health improvement programmes in line with the system healthcare inequalities strategy. You will ensure system approaches are based on local population need as defined by our population health management road map and integrated care strategy.

Main duties of the job

This is an exciting public health opportunity to play a critical role central to our ICB aims to improve outcomes in population health and tackle inequalities in outcomes, experience and access to healthcare.

You will lead delivery and monitoring of the System Healthcare Inequalities Strategy for Coventry and Warwickshire Integrated Care System linking closely to Geographical Care Collaboratives and aligning to our population health management approach.

You will provide public health oversight for a range of healthcare inequalities programmes, advising on planning, monitoring and evaluation.

About us

Coventry and Warwickshire Integrated Care Board (ICB) is responsible for commissioning NHS services on behalf of people in Coventry and Warwickshire. In consultation with local partners, it plans the delivery of services in a way that both meets local health and care needs and reduces inequalities between different groups. The purpose of our ICB is to bring partner organisations together to:

  1. improve outcomes in population health and healthcare
  2. tackle inequalities in outcomes, experience and access
  3. enhance productivity and value for money
  4. help the NHS support broader social and economic development

As an Integrated Care System we have agreed a common vision as well as the principles of how partners will work together. This is underpinned by theHealth and Wellbeing Concordatfor Coventry and Warwickshire.

This Concordat was agreed by our two Health and Wellbeing Boards and sets out our goal to enable people across Coventry and Warwickshire to pursue happy, healthy lives and put people and communities at the heart of everything we do.

Oversee the development, delivery and programme management of multiple programmes outlined within the System Healthcare Inequalities Strategy and Integrated Care Strategy aligned to local need and national policy

Development of evidence-based strategies to reduce health inequalities aligned to Core 20 Plus 5 Framework and major transformation programmes for example primary care development, elective care restoration, mental health transformation.

Lead evaluation of the Strategy and associated programmes, projects and redesign of services and pathways to ensure that any relevant learning is captured and incorporated into future programme delivery.

Be an expert and trainer on the implementation of HEAT (health equity assessment tool). Embed the use of HEAT across the system to tackle healthcare inequalities

Work with complex and varied teams to plan and deliver programmes to embed reducing health inequalities in our culture across the ICB

Develop local policies that promote best practice in reducing inequalities in health outcomes and access to health and care services, that reflect local need, national policy and align to local strategies
