This is an exciting opportunity to play a key role in the development and delivery of the Population Health Management (PHM) Programme for Hampshire and Isle of Wight Integrated Care Board. This is a pivotal role to the development and implementation of a high impact and high-profile programme in one of the most exciting new areas of health and wellbeing.
Ensuring patients receive the care they need, in the right place and at the right time is the top priority for the Hampshire and Isle of Wight Integrated Care System (ICS).

This is a hybrid role. The PHM team primarily work at home, but you will need to be able to attend face to face meetings within an office on at least a weekly basis.

You will be responsible for successfully bringing data into the new HealtheIntent population health management platform, supporting the development of robust processes and procedures for managing data, access and user management, providing technical expertise and supporting all technical aspects of the PHM programme. This is an exciting opportunity to accelerate the successes of the programme so far, being key to making the HealtheIntent platform the source of high-quality population health data in Hampshire and Isle of Wight.
Main duties of the job
You will have solid experience of data architecture and warehouse management, alongside project and change management methodologies, experience inputting into the development of policies, documents, plans and reports and experience working to tight and flexible timelines. This role may be perfect for those already in Data architect, Technical Lead, Junior Data Architect or JuniorTechnical Lead roles.
You will act as subject matter expert when working with our providers including hospitals, community and mental health, as well as other sources of data. You will need to will work closely with technical teams, paying close attention to detail and manage both expectations and time scales.
This is great opportunity to come and work within the friendly PHM programme team, as part of the wider partnership between HIOW PHM programme and Oracle Cerner and Optum. The ideal candidate will combine technical expertise and an attention to detail for relationship building combined with established project management skills. This role will require stakeholder engagement, internally and with wider stakeholders, therefore the candidate must be able to facilitate discussions and provide often complex, expert advice to colleagues as to how best practice might be adopted to support the uptake and delivery of PHM initiatives.
You will report to the Head of Population Health Management. You may be asked to line manage a small number of service desk team members.

Responsible for leading the technical workstream for the PHM programme; in particular:- Understanding the key data elements required to support the programmes prioritised use-cases- Reviewing the data available in existing data sources and identifying any gaps in data from the data sources that are planned to be onboarded- Continue to develop the PHM data onboarding strategy for the platform, carefully weighing up prioirities for onboarding of data sources into the platform based on local, regional and national prioirities- Engage with the data source owners to complete the data on-boarding readiness process to ensure they have the resources, capacity and technical capability to extract, load and validate the data- Act as the subject matter expert (SME) to support each data source lead through the data onboarding process linking in with the Cerner platform architects, data onboarding lead and data architects to understand how data is mapped into the data models- Support communication and engagement activities with end-users and PHM leads to ensure understanding of the data that is available and any limitations that may impact on use-case outputs- Line manage the Service Desk Manager responsible for providing second line technical support to users of the HealtheIntent platform- Work with Cerner and the testing lead to support the data source validation approach- Work with data source owners to define and establish the BAU service for on-going loading of data into the platform.- Lead on technical workstream service developments for the PHM programme and HealtheIntent platform- Collaborate with other technical leads on development of programme policies and standard operating procedures- Lead on maintenance and further development of high-quality user management processes- Within portfolio ensure high quality project management is employed, which includes producing and maintaining key project documents such as; project initiation document, highlight reports, project plans, risk and issue logs etc.- Regularly lead on research into PHM technical solutions, best practice, national policy and developments that would benefit HIOW HealtheIntent platform.
