We are looking for a dynamic and innovative clinician with experience working with CHC eligible adults with a range of complex needs to come and join our review team within the All Age Continuing Care and Specialist Placement team.

We provide a comprehensive and robust review to ensure commissioned services are meeting assessed needs and individuals continue to meet the criteria for NHS CHC. These range between home-based support, personal health budgets and nursing homes. You will be working across all health and social care sectors and visiting patients in their homes or placement. We work as a team across Kent and Medway. The role involves ensuring the work is appropriately planned and resourced, assisting the team with reviews, ensuring the reviews are high quality and robust, and applying clinical oversight for both the reviews and care management. The role requires good communication and assessment skills, the ability to promote team working, and a comprehensive understanding of the National Framework for Continuing Healthcare and Funded Nursing Care. Training on the specific framework will be given.

Main duties of the job

The main duties of the role are leading the review team, undertaking reviews, case management and the production of data for the review project. You will ratifying assessments and providing clinical oversight to the review team, as well as working with the wider CHC team. The role requires you to be confident in your assessment skills with strong clinical credibility.

We work closely with: Local Authority, Social care, providers of community services, Learning disability and autism services, hospices and Business as usual Continuing Health Care.

The role requires you to travel across Kent and Medway and to hospitals and placements out of area.

This role will require you to support and supervise junior staff and we work to a hybrid model whereby we work regularly in an office and from home.

All new starters with NHS Kent and Medway will be subject to a probationary period which is dependent on the length of contract. The probationary period works alongside the induction process to help create a positive and supportive working environment allowing new starters to learn the key elements of the role within a reasonable & realistic timescale.
