Job summary
This is an opportunity to work as part of the Learning Disability and Autism Programme with responsibility for
Chairing Care and Treatment Reviews and Care, Education and Treatment Reviews.
These reviews are part of NHS Englands commitment to transforming services for
people with learning disabilities, autism or both. You will chair both types of
reviews for people living in the community at risk of admission to hospital and
for in-patients in learning disability and mental health hospitals. These may
be online, hybrid or face to face so flexibility is a must, with a willingness
to travel (paid).
Our top priority is to reduce the number of people in inpatient settings by
supporting successful discharges and preventing inappropriate admissions. This
is not achievable without seeking to transform care in Kent and you will
therefore have opportunities to support in strategic change and development as
well as supporting good outcomes for the individuals at the heart of the
C(E)TRs. Your role would be varied with opportunity to learn as well as lead
and you will work with colleagues from multiple health and social care
organisations.
Main duties of the job
You would work within a small, friendly and committed team
which is part of a wider team, integrated with Kent County Council and Medway
Council.
Your main focus will be chairing Care (Education) and
Treatment Reviews and at times supporting multi disciplinary networks as a
critical friend and advisor.
Chairing includes an agreed allocation of actions to support
patients, so it is imperative that you have excellent communication and
confident people skills to enable effective partnership working. You may need
to challenge thinking, persuade practitioners and encourage different ways of
working to bring about change. To do this well you must also be a listener and
demonstrate understanding.
Lead on the design and completion of small scale projects to
transform and improve services in Kent
Attend meetings as set out in the main purpose of the job
description, to include the Dynamic Support Forums, patient resettlement
meetings, programme meetings, other regular system forums, and any necessary Commissioner
Oversight Visits to ensure compliance.

This post is an integral part of ensuring the
success of the Learning Disability and Autism (LDA) Programme and sets
expectations for reducing the number of in-patients with a learning disability
(LD) and / or autism across Kent and Medway.
The post holder will lead Care and Treatment
Reviews (CTRs) / Care, Education
and Treatment Reviews (CETRs)
as part of an independent panel on behalf of the Kent and Medway Integrated
Care System (ICS). The Integrated Care Board also has a duty to
complete Commissioner Oversight visits to those receiving treatment in
inpatient settings. You will be required to work as part of a team that ensures
all visits are completed and findings
from visits and reviews are connected and shared appropriately to move
forward successful and timely discharges from hospital.
The post holder will also
support the LDA Programme Management Office (PMO) with delivery of a range of
projects aimed at expanding and improving the range and quality of services and
support for people with LD or autism. This will involve leading on the design
and completion of small scale projects and supporting the design and delivery
of system level projects/initiatives.
This role will require the
post holder to work autonomously, supporting the management of challenging and
complex individuals, working in partnership with local and out of area
multidisciplinary teams to prevent hospital admissions and to successfully
discharge individuals from hospital to live in the place they call home;
proactively working to help them stay out of hospitals and specialist
residential provision, as appropriate.
The postholder will need to
have excellent people skills, well developed personal resilience and experience
of working within a complex public focused working environment. The postholder
will need to demonstrate transferrable skills acquired through their experience
of participating in multi-agency discussions or operating in a multi-agency
system. The successful candidate will have an ability to be flexible and
responsive to a wide range of people, patients, families partner agencies and
other stakeholders.
