We're looking for a highly motivated Registered Mental Health Nurse, Registered General Nurse, oran Allied Health Professional with relevant mental health patient experience with excellent communication and assessment skills to join our All Age Continuing Care and SpecialistPlacements Team (AACC), based at the Compass Centre in Chatham.
You'll be required to evidence aworking knowledge of NHS Continuing Healthcare pathways and tools and have an interest in developing your skills further inthis interesting field of practice.

Main duties of the job
This role requires a specialist Continuing Health Care (CHC) assessor to undertake and be responsible for completion of timelyand comprehensive assessments and to apply the relevant criteria to establish eligibility for services and care.You will complete any relevant assessment tools, for example the tools within the National Framework for NHSContinuing Healthcare and NHS Funded Nursing Care (FNC), in conjunction with individual /family and Multidisciplinary Team(MDT) members.You will:
- Case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required.
- Review and reassess individuals' eligibility for NHS CHC, FNC and any other NHS funded placements.
- Review any service, and care, commissioned in regard to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract.
- Complete retrospective reviews as required and directed by team manager.
- Be confident in presenting cases and making recommendations on eligibility and care/support required.
- Attend appeal/review panel meetings and present cases as appropriate.
- Participate in case conferences as required. Ensure accurate minutes and record of meetings are kept and distributed as appropriate.
- Participate in Best Interest Meetings as necessary.
- Ensure accurate and open communication with a range of organizations and individuals.
- Keep accurate and robust records at all times.
