Job summary

We have an exciting opportunity for a highly motivated Band 6 Registered Mental Health Nurse or Allied Health Professional to join our All Age Continuing Care and Specialist Placements Team (AACC), based at the Compass Centre in Chatham, Kent. You will be employed by NHS Kent and Medway Integrated Care Board (KMICB). You will be employed by KMICB and primarily working with patients originating from North Kent, placed in various geographical areas in England, but predominantly in Kent.

The mental health team within AACC is responsible for undertaking detailed clinical assessments, quality reviews and commissioning services and support for individuals subject to various NHS eligibility criteria, such as The Mental Health Act Section 117 aftercare, The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. The team will provide a variety of challenging and rewarding opportunities within a supportive and innovative environment.

We are seeking a Registered Mental Health Nurse or an Allied Health Professional with relevant mental health patient experience and with excellent communication and assessment skills.

Main duties of the job

oTo undertake timely, complex, and detailed assessments requiring high levels of concentration. This may include preparing reports, care and support plans for Section 117 PHBs.

o Prepare clear and carefully considered recommendations following review of eligible Section 117 cases.o Review individuals care and commissioned services, ensuring appropriateness to need, quality and value for the ICB.o Participate in team meetings and other meetings as required and relevant to the post.

o Demonstrate personal commitment to ongoing professional development commensurate with the requirements of the post

o Maintain legible, accurate and timely documentation at all times.

o Participate in Local Authority safeguarding reviews as required.

o Provide relevant and timely responses to requests for information, and guidance relevant to service.

o Work with members of the team and key stakeholders to ensure prompt delivery of the service within defined timescales.

o Have the ability to communicate tactfully and diplomatically contentious information to various individuals.

o Ability to communicate with individuals who may have a language, sensory or cognitive impairment

The hours are 37.5 per week and Monday to Friday 09.00 - 17.00 hours, though we can offer some flexibility.

It is important to note that you will need a full driving licence to undertake the requirements of this post.
