Main duties of the job

Our Community Nursing Service provides high-quality care to housebound adults with complex and long-term conditions. Working within teams aligned to local Primary Care Networks, we deliver integrated, proactive care that prevents avoidable hospital admissions and supports patients to achieve their long-term health goals.
As a Band 6 Community Nursing Team Leader, you will play a key role in leading and supporting the team to deliver excellent standards of care. You will supervise and develop staff through one-to-ones, appraisals, clinical supervision and return-to-work meetings, ensuring a positive and supportive learning environment. You will act as a senior clinician, developing integrated care plans with senior practitioners and commissioners, and guiding junior staff in assessing and managing patients with complex needs.
Your role will include undertaking a range of clinical and non-clinical interventions, prioritising caseloads, and contributing to service improvement and statutory requirements. You will work collaboratively with the wider MDT and external agencies to ensure seamless, patient-centred care
Additional Working Pattern: 7.5 hrs a day between Monday and Friday with late and weekends in rotation
