Job summary
We are looking for a Band 6 Physiotherapist to join our Maidstone based Pathway 3/Home First team team.
Pathway 3 is part of the Home First / Discharge to Assess Process, primarily in West Kent, with ongoing service developments to include opportunities to support East Kent services.
Pathway 3 supports discharge to local care home for individuals who require a further period of assessment to determine the long- term plan of care when an acute hospital admission is no longer required.

Main duties of the job
We work in collaboration with the acute trust Discharge Liaison Teams, Kent County Council Social Services Teams, All Age Continuing Health Care, local community services and organisations including the voluntary sector.
We work collaboratively to support the assessment of individuals in a timely way to determine their long-term need.
Communication and compassion is a fundamental part of the role, liaising with partner organisations, patients, relatives and carers is vital to ensure processes are understood and supported by all involved in the individuals care and assessment.
Pathway 3 support individuals during times that are often uncertain and extremely stressful, an empathetic approach is essential.
