Job summary
You will be a core member of the Urgent Care West Kent team based at Unit G, Maidstone where you will be delivering care, both clinical and non-clinical to patients/ clients. You'll also be key in supporting patients with their own self-care and independence. This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges. The service covers 24 hours a day 365 days a year. This service is providing nursing care within patients' homes and covering West Kent, the ability to drive is essential to this post. Under the umbrella of Urgent Care is the Home Treatment Service, which provides hospital level care at home. Part of this role would be supporting the HTS team.
Main duties of the job
Your daily activities will vary and include accurate patient note recording, basic observations, personal care (washing, dressing), catheter and stoma care, mouth care, supporting nutrition and hydration, blood glucose calibration, MRSA swabs, risk assessments (falls, skin integrity), subcutaneous injections, and venepuncture. You'll work in partnership with clients and families, ensuring consent, providing reassurance, and maintaining a professional, kind, and caring attitude.
This busy role requires teamwork within a multi-professional environment while supporting registered practitioners to deliver high-quality care. You may work independently but under indirect supervision, making informed decisions while knowing when to escalate concerns. Safeguarding people is a key responsibility, including recognising and responding to risks of abuse.
You'll contribute to delegated caseloads, engage in reflective learning, and continuous improvement through supervision, audits, and training. Supporting new colleagues, maintaining competencies, and adapting communication with kindness and compassion are fundamental to your work.

Essential
- A Qualifications and Credit Framework (QCF) Level 3.
- Full UK driving licence
- This could be an NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare or you will be able to demonstrate the equivalent level of experience.
- In addition you will also have Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C/ 4-9 or equivalent in English and Maths)
