Job summary

We are looking for a Cardiac Specialist Nurse to join our Community Cardiac Nursing Team based in Ramsgate.

We provide a specialist service to adult patients with complex cardiacconditions across East Kent, primarily heart failure. As part of the highly specialist nursing team, you will help support patients with chronic cardiac conditions that have been identified as having ongoing needs and would benefit from focused management.

Main duties of the job

Whilst patients are predominately seen in the clinic setting there is the ability to see patients in their homes where clinically indicated. You will undertake the assessment, delivery and evaluation of care required for this patient group. Provide the necessary health education and advice in order to achieve a reduction in the risk of further cardiac events, resulting in fewer hospital admissions, reduction in symptom burden and improved quality of life.

You'll undertake continuous professional development to enable the provision of pioneering specialist cardiac nursing services to meet the needs of the local population in line with multiple national and local targets.
