The pharmacy department within Kent Community Health NHS Foundation Trust are looking for Band 7 pharmacist colleagues who are enthusiastic and keen to develop their skills. We arelooking for pharmacists who are willing to work in different environments and thrive on thevariety of work a community Trust brings. You will also be given theopportunity to complete the clinical diploma and independent prescribing course if you have notalready done so. If you're willing there's opportunity to enhance your knowledge to develop theskills needed to excel in areas the role and receive training to become a Clinical Pharmacist witha focus on frailty.
Main duties of the job
As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to agreed community hospital sites. In addition, you will provide pharmaceutical advice to a range of specialist services.
You will develop an expertise in the optimisation of medicines in frailty, working with multidisciplinary teams to ensure that we achieve the best medicines related outcomes for the patients we serve.
You will be surprised at the wide range of services and healthcare professionals and what links we have working with acute Trusts, GPs PCNs and multidisciplinary teams.
