Job Title: Band 6 Community Nurse Team Leader
Location: Sevenoaks Community Hospital, Hospital Road, Sevenoaks, TN13 3PG.
Job Type: Full-time and Part-time options available.
We are looking for a dynamic and experienced Band 6 Community Nurse Team Leader to lead our community nursing team. The Team Leader will be responsible for overseeing the delivery of high-quality nursing care to patients in their homes or community settings. In addition to providing direct patient care, the Team Leader will also have supervisory responsibilities, including mentoring and supporting team members, coordinating care plans, and ensuring compliance with policies and procedures.
Qualifications:
- Registered Nurse with a valid nursing NMC pin.
- Experience in community nursing or a related field.
- Proven leadership and supervisory experience in a healthcare setting.
- Strong communication, organizational, and problem-solving skills.
- Ability to motivate and inspire team members to deliver high-quality care.
- Knowledge of community healthcare services and resources.
- Commitment to professional development and continuous improvement.
- Valid driving licence suitable to drive in the UK, plus access to your own car.

Main duties of the job
Responsibilities:
- Provide leadership and guidance to the community nursing team, including nurses and healthcare assistants.
- Supervise and support team members in the delivery of patient care, ensuring adherence to best practices and standards of care.
- Conduct regular team meetings, case reviews, and clinical supervision sessions.
- Collaborate with the multidisciplinary team to develop and implement care plans for patients.
- Monitor and evaluate the performance of team members, providing feedback and coaching as needed.
- Ensure compliance with policies, procedures, and regulatory requirements.
- Assist in the recruitment, training, and onboarding of new team members.
- Manage resources effectively to meet the needs of patients and the team.
- Act as a liaison between the community nursing team and other healthcare providers and organizations.
This role is fully supported by a team of experienced professionals, including the Senior Specialist Practitioner District Nurse, the Primary Care Network (PCN) lead, senior colleagues, and the practice education team. Their collective expertise and guidance provide invaluable support for the individual in this role, ensuring that they have access to mentorship, resources, and guidance to excel in their responsibilities. The Senior Specialist practitioner district nurse brings a wealth of knowledge and experience in district nursing, while the primary care network lead offers strategic direction and collaboration opportunities. Senior colleagues provide mentorship and insight from their own experiences, and the practice education team ensures ongoing learning and development opportunities. Together, this support network empowers the individual in this role to thrive and contribute effectively to the team and the organisation.
