Job summary
Urgent Care Services, comprising of The Acute Response Team (ART), Frailty, Virtual wards and EastKent Night Nursing Service (EKNNS), provides short term health, rehabilitation and care support toenable service users to remain in the community / home environment. This may include treatingacutely unwell people that would otherwise require hospital admission.
Care provided by the EKNNS team includes End of life care, catheter care and other urgent care that is unplanned and cannot wait until the following day.
Home is nationally evidenced to be the best place to recover and regain independence. Enablementand rehabilitation will be the priority focus with active participation from service users in order toachieve maximum independence.
We are working with East Kent Hospitals University Trust, who are leading on developing a virtual ward for people with acute respiratory infections, such as a chest infection, or with anunderlying condition, like COPD, to be cared for at home.
Patients who agree to be discharged earlier from hospital, will be monitored at home usingtechnology, such as apps, wearables or other medical devices remotely - and supported by aconsultant, specialist GP or advanced practitioner. What technology will be used is still being scopedas we co-design the wards with patients and colleagues during the months ahead.

Main duties of the job
Caseload Assistant will be aligned to a team but might have to travel to other areas should thedemand require it. Key responsibilities will include:
- Liaise with ART Band 4 Coordinator with regards to referrals and patient discharges from the acute and community hospital sites into the Acute Response Team
- Answer the Duty phone and address daily service queries such as change of visits/patient enquiries/messages for staff- escalate triaging and clinical enquires to Duty Clinician
- Autonomous daily support for Duty Clinician with administration of HCA clinical handover tasks including transferring of sensitive and confidential patient information to clinical notes system.Observing all confidentiality protocols in place.
- Undertakes a defined role in data collection to inform KPI reporting
- Daily allocation of patients visits to HCAs, which will require analysis of current and projected rostering information.
- Address and escalate blocks to caseload flow with regards to staff allocation/availability.
