Job summary
This post is for a Speciality Doctor for management of patients in the West Kent community hospitals and the Home Treatment Service. It may include other community hospitals and patients in other community settings such as nursing homes and their own homes.
The community hospitals treat patients for rehab as step down from acute hospitals and step up care with more acutely unwell but stable patients from their own homes. This includes the Home First programme (Discharge to assess).
The post is permanent and will cover Tonbridge, Sevenoaks, Edenbridge and Hawkhurst community hospitals, but work in other hospitals and care setting will be part of the role. The Home Treatment Service (HTS) was established to provide hospital at home level care and takes patients for both hospital avoidance and for facilitated discharge. Work is underway to integrate services across Kent as part of the STP. In West Kent frailty services are under development, both in the Community and in the Acute Trust and it is anticipated that this post will participate in future frailty pathway work. This post will provide varied experience and the opportunity for personal development.
The post is for 40 hours per week and will include some weekend and evening working. Part time working may be considered.
Cover for leave will be arranged on a cross cover arrangement with the other Doctors in the Directorate. This will mean covering other sites on occasion.
Main duties of the job
The role will be supported on a sessional basis by Consultant Geriatricians be responsible whilst on duty for the medical care of patients in beds at relevant Community Hospitals be responsible whilst on duty for medical care of patients under the Home Treatment Service.
Be responsible for ensuring continuity of medical care to patients, providing an appropriate medical handover to the out of hours service and receiving medical handover from the out of hours doctor where appropriate
Be a key member of the clinical team and participate fully in the clinical team meetings, strategic discussions and regular ward rounds
Support development of all ward staff and participate in the development plan
Contribute to the patient care programme and monitor patients' progress towards those objectives
Liaise and work closely with:
o Adult Services Teams
o Other Doctors and care staff within the Trust
o Community Matrons, ACPs, Complex Care Nurses and other staff
o The Community Rehabilitation Services
o Social Services Teams
o General Practitioners
