Job summary
Are you motivated and experienced practioner with an interest in falls prevention andsupporting people who are at risk of falls? If so and you are self-directed and responsibleclinician we need you for our community falls prevention service in sunny West Kent.
We're dynamic team of practitioners including Occupational Therapist's, Physios,Therapy assistants and admin staff who work on service model for patients who have fallenor who are at risk of falling.
We aim to reduce the risk of falls, to improve outcomes for those who do fall and to help patients achieve maximum independence in daily living activities. This improves patient well-being, reduces admissions and reduces personal and financial costs in the long term.
Main duties of the job
Therapy assistant work closely with Occupational Therapist's and Physiotherapists.
You will work on the assessment of falls risk factors and the review of prescribed exercises programme. Your role will also include assessment and provision of basic equipment and home adaptations.
The job base ranges from Coxheath/Tonbridge localities, depending upon service needs. Youwill be part of a great team and you will be free to take autonomy of your own caseload and management.
You will need to be able to move from base in a timely manner using own transport.
