We are looking for a dedicated Health and Social Care Coordinator to join our team in Coxheath.
As part of our Health and Social Care Coordinator Team in West Kent you will support adults with long term conditions living primarily in the Maidstone South, Headcorn, Harrietsham & Lenham areas by enabling them to access and engage with wider health and social care sectors to improve their overall health and wellbeing, by aiming to improve self management and unnecessary hospital admissions due to crisis.
Main duties of the job
You will need strong communication skills as you will be working with all members of the multi-disciplinary team to meet the needs of vulnerable and high-risk patients within their caseloads.
You will have telephone contact with patients and will also conduct face to face home visits, and clinics where you will assess their needs and support them by signposting them to the agencies that can help them to improve or maintain their quality of life and wellbeing. A broad knowledge of Health and Social Care services is necessary, as is an understanding of the diverse opportunities offered by the voluntary sector in this locality.
We work very closely with our Clinical Commissioning Group and the successful candidate would need to be flexible and adapt to the changing requirements of the service as it evolves.
As part of your role you would be required to evidence your work to support evaluation of the service and participate in social prescribing for patients. Excellent IT skills are required for this post.
