Job summary

We currently have an exciting new created post for a Falls Prevention team lead therapist to manage a well-established West Kent falls Prevention service and Postural stability Instructor (PSI) service in the East Kent. This post is open to applications HCPC / NMC registrant's.

We are amulti-disciplinary teamthat includes consultants, occupational therapists, physiotherapists and therapy assistants. We provide holistic assessment of falls risk factors including physical, environmental and medical factors. We work within our clinics, patients' homes and rehabilitation groups to ensure a seamless assessment and rehabilitation pathway.

We aim to reduce the risk of falls, to improve outcomes for those who do fall and to help patients achieve maximum independence in daily living activities. This improves patient well-being, reducesadmissions and reduces personal and financial costs in the long term.

You will be mainly Coxheath based but will be working across locations.

Main duties of the job

In this newly created post, you will be required to deliver Falls Prevention pathways of care in accordance with the Trust Quality Priorities and nationally and locally agreed standards.

You will provide an expert evidence-based falls prevention support to the falls prevention and the other services across West Kent.

You will be expected to be a visible leader in the clinical setting, able to move between locations time efficiently (Car driver) and placing the clinical environment at the heart of learning. Post holder to be able to manage clinical competencies, clinical supervisions, waiting list management and complex case management team meetings.

You will be working within the principles of continuous improvement they will review, implement and monitor adherence to the Trust falls policy. Communicating best practice and supporting areas to share improvements.
