We are looking for a dynamic, motivated and enthusiastic experienced diabetes nurse/practitioner to join Kent Community Health NHS Foundation Trust to help us deliver patient focused improvements in diabetes care, while supporting innovation and service development in the primary care setting. The NHS Long Term Plan sets out a range of improvements for those at risk of Type 2 diabetes and those living with Type 1 and Type 2 diabetes. The focus of this exciting new role is being committed to the development of innovative practice and expanding evidence-based care which will contribute to the enhancement of professionals working within primary care and the Health Care Partnerships (HCPs). We are looking for a nurse/practitioner with experience and non-medical prescribing in diabetes care, across any of the specialist care pathways. Main duties of the job
The post holder will be an experienced diabetes practitioner, reporting to the Diabetes Advanced Practitioner. Acting within professional boundaries, you will provide support to primary care colleagues, by furthering professional development in the area of diabetes to support high standards of clinical assessment, competencies and management of patients attending primary care. The successful post holder will work within the Primary Care Networks (PCNs), establish and links with other diabetes service teams, that provides specialist diabetes care across integrated clinical pathways. Working as part of the integrated team there will be opportunity to further develop diabetes skills across primary care.\nThe posts are full time; however, 30 hours would be considered.\nWorking hours are 9-5, Monday to Friday
