Job summary

An exciting opportunity has arisen for a SEND practitioner to come and join the 0-19 Healthy Child service in St Helens working for Wirral Community Health and Care NHS Foundation Trust.

The post requires good communication and interpersonal skills and the ability to be adaptable, reliable, compassionate, and approachable. You will need to have strong interpersonal and leadership skills, be flexible solution focused and support service development.

You will work as part of an integrated 0-19+ Team and be a SEND champion for the service.

As a key stakeholder in the protection of children, you willl be required to work collaboratively with other agencies to meet their needs. You will have a good knowledge of the components of Clinical Governance, Risk Management, Audits, and relevant Professional Codes.

Main duties of the job

To be a SEND Champion for the wider 0-19 service and provide advice and guidance to support Public Health Nurses with their caseload of children with SEND or additional needs.

To identify and prioritise the health and care needs of children, young people and their families, working in collaboration with people throughout the care planning process.

To support all services within the Integrated Children's Division to ensure that the needs of children and young people with SEND (Special Educational Needs and Disabilities) and EHCP's are met.

To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local needs, in relation to aims and objectives of Wirral Community Health and Care NHS Foundation Trust.

To develop effective links with key people in delivering services to enable people to improve their own health.

To contribute to safe, effective services and provide children and families with a positive experience of our Service.

Essential

  • First level registered nurse current with NMC or registered healthcare professional with the HCPC (Allied Health Professional)
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
  • Post graduate qualification in children's services or equivalent experience.
  • Evidence of continued professional development

Experience & Knowledge

Essential

  • Knowledge of SEND reforms
  • Demonstrate a sound knowledge of service priorities
  • Experience of working collaboratively with other agencies and professions to deliver change
  • Experience in presenting and leading groups
  • Experience of patient and staff engagement to improve service delivery
