Job summary

Are you an enthusiastic, self-motivated Nurse wishing to join the proactive, dynamic and passionate Community Cardiology team at Wirral Community Health and Care NHSFT.

We need an experienced clinical nurse specialist who can work well both autonomously as well as part of a wider team. A qualification in non-medical prescribing, clinical examination skills and clinical diagnostics are essential to meet the services needs.

Providing specialist services in the community is key to enable early diagnosis of CVD related conditions and effective clinical management of this health population. Ensuring patients have access to medical optimisation, cardiac interventions and CVD Rehabilitation to improve quality of life, clinical outcomes and reduce the risk of complications leading to unplanned hospital admissions.

Main duties of the job

  • Work as part of a Multidisciplinary team of Cardiology Consultants and GPs with Specialist interest in Cardiology, Specialist Nurses and Exercise physiologists to provide early diagnosis and complex management of cardiac conditions including coronary heart disease, valvular disease and arrhythmias.
  • The role requires the assessment and evaluation of patients with a diagnosis of cardiac/cardiovascular conditions to enable treatment plans to be implemented and hospital admission avoided.
  • To provide specialist cardiac/cardiovascular care advice and education to clinical colleagues (multidisciplinary team) and patients/carers/relatives.
  • The post-holder will have line management responsibility for the Clinical technicians in the community cardiology team.
