Job summary

An opportunity has arisen to join our new Home First approach Team, based with the Community Integrated Response Team. This is a system level collaboration between WUTH, WCHC, Wirral Borough Council and Healthy Wirral designed to support timely discharges and complete therapy assessment and intervention in patients own homes.

The roles will work across Frailty Virtual Wards, Urgent Community Response and Home First.

Main duties of the job

We are looking for an enthusiastic and forward thinking care coordinator to join our busy, pro-active, integrated services based within Pennant House, Bebington, Wirral

The role will involve the coordination of health care assistants and clinical support workers who will be delivering reablement and domiciliary care interventions in the patients home environment. You will liaise with discharge trackers to facilitate timely and safe discharge., working with the multidisciplinary team to ensure best possible outcomes are achieved for patients, their carers and families.

You need to have significant NHS experience, preferably within a community setting, have a good understanding of risk management, rostering and scheduling care. Excellent communication skills and an ability to prioritise discharges of complex patients are essential. You will need good time management skills.

You will manage a team facilitating team meetings, supervisions and all HR processes. There will be the opportunity for supervision and to attend in-service training and professional development.

About us

Appointment to the post will be subject to successful interview, references, DBS clearance and Occupational Health checks.

The service will operate with extended hours 7 days per week providing cover 8am-8pm. You will be expected to work to cover service demands across the extended hours and weekends on a roster basis.

Qualifications and professional training

Essential

  • NVQ Level 3 in care management or equivalent health and social care discipline.
  • Evidence of Continuing Professional Development (CPD).

Desirable

  • ECDL or equivalent ICT qualification.

Experience and knowledge

Essential

  • Broad practical and theoretical understanding of a particular service area generally based on a combination of formal academic study and/or substantial relevant practical management experience.
  • 3 years Proven experience of working in a Health or Social care environment role
  • Evidence of understanding / complexity of issues locally and nationally pertaining to management of adult with complex health conditions.
  • Experience of working independently, unsupervised providing high quality care.
  • Evidence of working in multi-disciplinary services.
