Main duties of the job
An opportunity has arisen for a dynamic and motivated, experienced, band 6 Social worker to join a busy, pro active, multi- disciplinaryteam within the Trust.
Full and part time vacancies are available in:
- Community (Integrated Community Care Teams)
- Multi Agency Safeguarding Hub (MASH)
You will work with people aged 18+ who may have multiple conditions, learning disabilities and complex care needs as part of a multi disciplinary team in a community setting.
To join our Community Social Work teams you will:
- Be a registered Social Worker with Social Work England
- Have completed ASYE and critical reflection modules post qualification
- Have proven knowledge of the Care Act 2014, Mental Capacity act 2005, Safeguarding and other specific legislation appropriate to Adult Social care
- Be committed to delivering a high standard of care and promoting professional standards
- Be an excellent communicator who will innovate, inspire and support colleagues
- Advocate for members of our community, identifying changes in need and planning for their current and future care
This post requires applicants to be at a social worker level having completed your ASYE.

Experience and Knowladge

- Able to demonstrate practice which supports the core social work values and principles.
- Proven experience of working with vulnerable adults and carers.
- Knowledge of legislation relevant to Adult Social Care and work
- Understanding of confidentiality and data protection requirements.
- Experience of completing safeguarding enquires and making safeguarding personal. Undertaking statutory duties and coordinating
