A unique vacancy has arisen for a dynamic Nursing Healthcare Assistant to join our Community Nursing Team in Chesterfield.

The role of the HCA is pivotal to delivering excellence in Community Nursing Care and the successful candidate is expected to be supported in delivering hands on care to our patients autonomously. A Full competency framework and training will be provided. You will have the guidance and support of a team of Registered Nursing professionals and Clinical Practice Facilitator's whilst undertaking an expanded scope of duties.

Duties may include:

Medication administration

Diabetes Care, advice and education

Catheter care

Venepuncture

Double up visits and Patient Manual Handling support visits

Simple wound care

Leg ulcer care and some compression bandaging

Completing patients clinical observations when indicated and escalating care appropriately, with guidance from senior registered team members.

Please note, a valid UK driving licence is essential for this role.

Main duties of the job

To maintain standards, ensuring quality of patient care through the development of a range of knowledge & competencies to support Clinicians. To conduct a range of planned community nursing intervention in patients own homes.

To support and ensure the effective seamless flow of patients through the Community Nursing Caseload.

To assist Clinicians with the provision of a high standard of individualised patient care and treatments where appropriate and work as an effective multi-disciplinary team member.

To carry out individual patient care within patients own homes without direct supervision, under the direction / care planning of a registered clinician / clinician-in-charge.

Understands and observes appropriate standards of confidentiality.

At all times to behave and act in a way appropriate to and in accordance with "The DCHS Way".
